* feat: fix and re-enable Azure Portal pipeline metrics dashboard

Update dashboard.bicep to work with App Insights (no Log Analytics):
- Change param from logAnalyticsWorkspaceResourceId to appInsightsResourceId
- Update API version to 2022-12-01-preview
- Change resourceTypeMode from workspace to components
- Fix KQL queries: remove copilot.* prefix filter (services use
  service-specific prefixes like parsing.*, chunking.*, embedding.*)
- Add 3 new tiles: Pipeline Throughput, LLM Token Usage, Retry Rates

Wire dashboard module back into main.bicep with appInsightsId from
azureMonitorModule and uncomment dashboard outputs.

Update DASHBOARD.md with correct metric naming and new tile descriptions.

- Split LLM Token Usage KQL by customDimensions.type (prompt vs completion)
- Add service prefix to Pipeline Throughput metric names in docs
- Add service prefix to LLM Token Usage metric names in docs
- Fix example snippet resourceTypeMode from workspace to components
