fix: include stderr content in codex error messages on non-zero exit

Previously, when codex exited with non-zero status, only "exit status 1"
was shown with no context. Now the last 5 stderr lines (truncated to
256 chars each) are included in the error message, surfacing actual
failure reasons like authentication errors or rate limits.
