chore: exclude irrelevant modules from coverage report (#855)

Skip files that don't make sense to track coverage for, and were skewing
the metric:

- Forked/vendored code (libcluster postgres strategy, PromEx phoenix
dashboard)
- Supervisor/application modules (application.ex, telemetry.ex)
- Library delegation modules (cloak vault, ecto repo, phoenix endpoint)
- Helper macro modules (supavisor_web.ex)
- Release tasks (release.ex) meant to run outside the application
