docs: Add RTMX Patterns and Anti-Patterns guide

Create single-source-of-truth patterns document that:
- Defines closed-loop verification as core principle
- Documents verification, development, and agent patterns
- Provides anti-pattern examples and quick reference

Update CLAUDE.md with patterns summary and link.
Update agent install prompts to include patterns URL and
critical warning about closed-loop verification.

This addresses the anti-pattern where agents manually edit
RTM status instead of using `rtmx verify --update`.
