* fix(logger): replace shared channels with per-invocation context cancellation

The shared unbuffered Done/Fail channels caused cross-waking between
concurrent Loading() goroutines, goroutine leaks when signals were
missed, and panic-on-close-then-send in Exit(). Each Loading() now
returns a context.CancelCauseFunc for independent lifecycle control.

Addresses RISK 3 from PROJECT_360_EVALUATION.md.
