feat(docker): use CLAUDE_CONFIG_DIR env var for alternate Claude installations

Add support for CLAUDE_CONFIG_DIR environment variable in the Docker
wrapper to select alternate Claude config directories. Derives macOS
Keychain service name from the config path for credential extraction.
Update README with env var documentation and FAQ entry.
