fix: ensure processes are visible for hot code upgrades (#850)

The child_spec was missing `type: :supervisor`, causing the release
handler to treat TenantSupervisor as a worker and skip its children
during hot upgrades.

Additionally, poolboy needed a patch to work with hot code upgrades.
