feat(mcp): HTTP transport failover and reconnect

Session management per MCP spec: first connect without session ID; store server-assigned session for subsequent requests.

On 404 or 'Session not found', reconnect (clear session, connect again) and retry once in callMcp and getMcpTools.

Fallback for servers requiring session ID on first request: retry connect with client-generated session ID.

Recover client when missing (e.g. after server restart) by attempting connect on first tool use when config exists.
