fix: remove redundant startup requeue from summarization service (#1137)

The summarization service's startup requeue bypassed the orchestrator,
using a basic default prompt template and no pre-selected chunks. This
produced duplicate, lower-quality summaries for every thread.

The orchestrator already has its own startup requeue (lines 180-260)
that uses the correct structured prompt, context selector, and dedup
check. The summarization service's requeue was fully redundant.

Removes _requeue_incomplete_threads() and the enable_startup_requeue
parameter from SummarizationService.start(), along with all associated
tests in test_forward_progress.py.

Fixes #1136
