Clean up server package deprecated code and tests

Removes backward compatibility shims and updates tests:
- Remove addNoteRequest.getContent() helper, use req.Note directly
- Remove legacy Content field from addNoteRequest struct
- Update handleAddNote to use req.Note instead of getContent()
- Remove formatPct template alias (use formatPercent)
- Update label tests to remove Content field from SourceInfo

Simplifies server code by eliminating legacy field compatibility layers.
