refactor: remove SQLX_OFFLINE requirement from builds and tests (#935)

Replace compile-time checked sqlx query macros with runtime query_as
calls in cost_model.rs, removing the need for SQLX_OFFLINE=true in
all Dockerfiles. Update documentation to reflect the simplified
build and test workflow.
