Add toolhive-release skill for automated release workflow (#3549)

* Add toolhive-release skill for automated release workflow

Add a Claude Code skill that automates the ToolHive release process:
- Analyzes commits since last release
- Categorizes changes (features, fixes, breaking changes, etc.)
- Recommends semantic version bump type (major/minor/patch)
- Triggers the create-release-pr workflow
- Monitors and reports the created PR

Includes reference documentation for the complete release workflow chain.

* Use GitHub MCP tools instead of gh CLI in release skill

Replace gh CLI commands with GitHub MCP tool calls for better
integration and reliability:
- gh workflow run → mcp__github__run_workflow
- gh run list → mcp__github__list_workflow_runs
- gh run watch → mcp__github__get_workflow_run (polling)
- gh pr list → mcp__github__list_pull_requests

Also adds explicit user confirmation step before triggering
workflows and improved error handling guidance.
