DAO Indexes (GSI-2126) (#213)

* Add index support for DAOs

* Add index violation error and explicit list of index properties, plus tests

* Bump version to 8.0.0

* Remove list of properties because I just saw it wasn't an exhaustive list

* Add better documentation to MongoDbIndex

* Make sure we can include the ID field in indexes

* Rework error classes

* Simplify list construction
