Max coverage (#481)

* test(req): add tests to achieve 100% coverage for fresh.ts

Add 8 new tests covering:
- Weak ETag comparisons (W/ prefix on request and response)
- Comma-separated ETags in If-None-Match header
- Non-matching comma-separated ETags
- Leading spaces in If-None-Match
- Spaces between comma-separated ETags
- Trailing spaces after ETag values
- lastModified <= modifiedSince returning true

Add tests covering:
- x-forwarded-host when proxy is trusted
- Array :authority header returning undefined
- Array x-forwarded-host header returning undefined
- Matching :authority and host headers
- IPv6 literal in host header parsing
- Hostname without port
- Subdomains empty array when hostname undefined
- IP address as subdomain when hostname is an IP

Improves request.ts coverage:
- Statements: 86% -> 97%
- Branches: 72% -> 93%
- Lines: 96% -> 100%

Add tests covering:
- max as an async function
- Successful requests with skipFailedRequests (no decrement)
- Failed requests with skipSuccessfulRequests (no decrement)
- onLimitReached callback when limit is first reached
- Store error in incr callback
- headers: false option when rate limited

Improves rate-limit.ts coverage:
- Statements: 95% -> 97%
- Branches: 83% -> 94%
