feat: add background to GitHub Actions icons

Enhance the visibility of GitHub Actions status icons by layering
them over a white circular background.

* Refactor icon rendering into a layered **Box** structure.
* Ensure a fallback **unknown** status is used instead of `null`.
* Move icon selection logic to a standalone `getIcon` function.
