Fix all xo linting issues without breaking tests

- Fixed import ordering and spacing issues via xo --fix
- Added proper process import from node:process
- Fixed naming convention issues with eslint-disable for legitimate cases (env vars, API fields, constants)
- Fixed nullish coalescing operator usage
- Removed unnecessary default cases from exhaustive switches
- Added eslint-disable for intentional patterns (await-in-loop, non-exhaustive switches, deprecated APIs)
- Fixed Array.push multiple calls, string concatenation, and other code style issues
- All 98 tests pass successfully
