refactor(input): consolidate constructors and extract stdin/stdout helpers

- replace NewTerminalCollector() and NewTerminalCollectorWithOptions(noColor)
  with single NewTerminalCollector(noColor bool)
- add getStdin() and getStdout() helper methods to eliminate duplicate
  nil-check initialization blocks in selectWithNumbers and AskDraftReview
- update tests and caller in main.go
