Add Phoenix LiveDashboard page for Oban health metrics

Adds an optional LiveDashboard page that visualizes health metrics from
the existing TableHealth, IndexHealth, and QueueMetrics plugins.

Features:
- Summary cards for table, index, and queue health
- Alerts section showing active warnings from health checks
- Queue details table with job counts and utilization
- Table details with size, dead tuples, vacuum status
- Index details with sizes, scan counts, and status

The page is configured via router options and supports multiple Oban
instances with custom titles.

Requires phoenix_live_dashboard ~> 0.8 (optional dependency).
