* feat: add Claude Code plugin infrastructure with marketplace support

- Add .claude-plugin/ with plugin.json, marketplace.json, and README
- Convert skills to directory structure with symlinks (ralphex, ralphex-plan)
- Add version sync script for goreleaser pipeline
- Update documentation with plugin installation instructions
- Add gitignore rule for plugin version sync backups

* fix: add CLI installation check to skills

Add `which ralphex` verification to both skill files with installation
guidance if missing, similar to llms.txt pattern. The /ralphex skill
blocks until CLI is installed, while /ralphex-plan proceeds with
plan creation but reminds user to install before execution.

Addresses PR review feedback.
