feat(feature/batch-processing): Add concurrent batch processing with Task.async_stream (Step 13)

- New function: ExOutlines.generate_batch/2 for concurrent generation
- Batch options: max_concurrency, timeout, on_timeout, ordered
- Leverages BEAM's Task.async_stream for efficient concurrency
- Telemetry events: [:ex_outlines, :batch, :start] and [:ex_outlines, :batch, :stop]
- Success/error count tracking in telemetry
- Independent task execution (failures don't stop batch)
- 18 comprehensive tests covering all batch scenarios
- All 261 tests passing
