Add startup/readiness probes for ACA services (#1051)

* feat: add startup/readiness probes for ACA services

Add /readyz endpoint to chunking, embedding, orchestrator, and summarization
services. Configure startup and readiness probes in containerapps.bicep to
prevent false probe failures during slow startup (e.g., Cosmos/Service Bus
connection initialization).

- Startup probes: 60-120s grace period depending on service complexity
- Readiness probes: check /readyz every 10s, requires subscriber thread running
- No memory changes (investigating root cause separately)

Supersedes #1028 (probe-only changes without memory increases)

* fix: Correct service constructor parameters in test fixtures

- orchestrator/tests/test_api.py: Remove invalid context_selector param,
  add vector_store and prompt file paths
- summarization/tests/test_api.py: Replace invalid llm_backend and
  embedding_backend with required summarizer param

Addresses review comments about invalid keyword arguments.
