Refactored Keymaster with mixins

Extracted 8 feature mixins from keymaster.ts (3689 → 1680 lines):
- credential-mixin.ts (606 lines) - credentials + challenges
- poll-mixin.ts (411 lines) - polling functionality
- vault-mixin.ts (375 lines) - encrypted vaults
- dmail-mixin.ts (262 lines) - encrypted messaging
- notice-mixin.ts (215 lines) - notification handling
- group-mixin.ts (186 lines) - group management
- document-mixin.ts (173 lines) - image/file assets
- schema-mixin.ts (163 lines) - JSON schema handling

All 648 tests pass.
