improve coverage for plan creation mode:
- cmd/ralphex: add tests for setupRunnerLogger, getCurrentBranch,
  setupGitForExecution, handlePostExecution, validateFlags,
  printStartupInfo, findRecentPlan
- pkg/processor: add direct test for buildPlanPrompt

coverage improved from 37.6% to 50.4% for cmd/ralphex
