refactor(config): split into focused modules with local config support

- split config.go into values.go, colors.go, prompts.go, agents.go, defaults.go
- add local project config (.ralphex/) with precedence over global config
- implement per-field merge for config values (local > global > embedded)
- implement per-file fallback for prompts (local > global > embedded)
- implement replace strategy for agents (local replaces global entirely)
- add *Set flags to track explicit false/0 values in config
- add validation for negative integer fields (codex_timeout_ms, iteration_delay_ms)
- wrap prompt loading errors with descriptive context
- update precedence comment in defaults/config
