qt: add Changelog to Help menu in toolbar

Adds a `Changelog` item to the Help menu of the toolbar.
The dialog reads and shows the RELEASE-NOTES file or shows a link to the
release notes on github as fallback.

I feel like accessing the changelog through this path is a bit hacky but
not sure if there is a better alternative. We could also download the
changelog from github but then it relies on some webserver.
