feat: use Heroicons for GFM alert icons

Replace default octicon SVGs with Heroicons (mini, 20x20) for
markdown alerts:

- Note: information-circle
- Tip: light-bulb
- Important: exclamation-circle
- Warning: exclamation-triangle
- Caution: x-circle

Bumps version to 0.11.1
