Improve mnemonic name generator with diphthongs and Q handling

- Use diphthongs (ai, oo, ei) for inserted vowels to distinguish from
  original amino acid vowels (A, E, I, Y)
- Q followed by 'o' when not followed by a/e/i/o for natural pronunciation
- Separate consonant cluster rules for word start, middle, and end
- Consonant-aware vowel selection based on phonetic affinity
- Min 3-char words to avoid awkward short endings
- Similar sequences produce similar names for easy clonotype grouping
- Update tests and README with new examples
