feat: Add SSRF protection

Block private and sensitive networks with an option to enable access.
Enable access to localhost by default with an option to also disable.

  - Private networks (10.x, 172.x, 192.168.x) blocked by default
  - Cloud metadata (169.254.x) always blocked unless private enabled
  - Add MD_SERVER_ALLOW_LOCALHOST and MD_SERVER_ALLOW_PRIVATE_NETWORKS
