feat: add clientRef prop to providers for parent component access

Add clientRef prop to FathomProvider and NativeFathomProvider that allows
the parent component (which composes the provider) to access the client
instance directly. This solves the problem where the parent cannot use
useFathom() since React context only flows downward to children.

This is particularly useful for:
- Tracking events from app-level handlers (deep links, navigation, etc.)
- Accessing the WebViewFathomClient's queue management methods in React Native
- Any scenario where the composing component needs to fire commands
