fix: pre-commit がファイルを修正した場合に自動リトライする機能を追加

- _run_git_commit に pre-commit フックによるファイル修正検出を追加
- "files were modified by this hook" を検出して自動リトライ
- git add -u で全変更ファイルを再ステージング
- 最大3回までリトライ（無限ループ防止）
- テストを追加（test_run_git_commit_precommit_retry, test_run_git_commit_precommit_retry_max_retries）
