feat(form): add configurable form data collection

Add form flow feature for collecting structured data from users:
- Form config in chatParams.form with intro, end, template, items
- Text fields extracted via LLM (form-extractor agent)
- Button fields via inline keyboard (callback_data with indices)
- Collected data sent to specified recipients via message_template
- Form state stored in thread, resets after completion
