fix: configure Kamal to use port 80 and extend healthcheck timeouts

- Set proxy app_port to 80 (matches Thruster configuration)
- Configure healthcheck to check port 80 instead of default 3000
- Extend deploy_timeout from 150s to 300s
- Add healthcheck_timeout of 180s
- Increase healthcheck max_attempts to 15
- Extend healthcheck interval and timeout for slower startups

This should resolve the 'connection refused' errors where Kamal proxy
was trying to connect to port 3000 while the app runs on port 80.
