Implement ADR-0007 Phase 3: Formatter interface with JSON, JSONL, and text formatters (#75)

- Added FormatNode to AST for configurable foreach output formatting
- Implemented Formatter interface for result serialization
- Created JSONFormatter: returns Go array for downstream JSON encoding
- Created JSONLFormatter: returns newline-delimited JSON string
- Created TextFormatter: concatenates results with configurable delimiter
- Added NewFormatter factory function with format type validation
- Comprehensive unit tests with >90% coverage
- All formatters handle empty arrays, mixed types, and edge cases
