feat: add `new_term_index()` convenience method to `LogId`

Add a convenience method for creating `LogId` directly from term and index
when using standard Raft (`leader_id_std::LeaderId`). This eliminates
the need to manually wrap terms in `CommittedLeaderId`.

Changes:
- Add `LogId::new_term_index(term, index)` method for std LeaderId configs
- Add tests for `new_term_index()` functionality
