feat: Implement REQ-UX-001 and REQ-UX-006 rich progress bars

Phase 5 CLI UX enhancements:
- Add rich library as optional dependency (rtmx[rich])
- Implement rich progress bars with colored panels
- Fixed-width 70-column panels for consistent display
- Color-coded progress bars matching legend (green/yellow/red)
- Graceful fallback to plain output without rich
- Add --rich/--no-rich CLI flags for explicit control

Tests: 8 new tests in tests/test_cli_ux.py
