feat: Add tool annotations for improved LLM tool understanding (#3167)

Add readOnlyHint and destructiveHint annotations to all 8 MCP tools
to help LLMs better understand tool behavior and make safer decisions.

Changes:
- Added readOnlyHint: true to read-only tools (search_registry, list_servers,
  get_server_logs, list_secrets)
- Added destructiveHint: true to tools that modify state (run_server,
  stop_server, remove_server, set_secret)
- Added title annotations for human-readable display

This improves tool safety metadata for MCP clients.
