* fix: address MCP server issues and improve tool definitions

- Add error handling to all MCP tool implementations
- Fix label filtering logic in remove_labels
- Add error handling to runMcpServer to prevent crash risks
- Remove unused --port and --provider options from CLI
- Fix incorrect tool mappings in src/tools/index.ts
- Add listIssuesTool to src/tools/issue.ts
- Run biome check --write
