Docs for Desired Error Handling (#3089)

Summary
This PR introduces docs/error-handling.md and modifies CLAUDE.md to describe our desired way of handling, manipulating, and logging errors.

Closes #2765

Follow-up Work
The documentation is aspirational. I've fleshed out #2766 to describe how we'll close the gap between where we currently are and the end state described by this doc.
