docs(pages): update lazy examples to use Provider.fromClass instead of Registration

Remove raw function examples (p) => p.lazy() and focus only on the lazy() helper.

Changes:
- Updated docs/src/pages/provider.mdx to show Provider.fromClass().pipe(lazy())
- Removed raw function example from documentation
- Updated test to remove duplicate test case using raw function
- Updated README template description for clarity
