New Operator.Slice (#413)

* Bump dev dependencies

* Account for Operator.Slice in Integrity and Packing

* Implement Operator.Slice for extracting portions of dynamic bytes

* Use shift/size nomenclature consistently for Slice operator

* Move Operator.Slice to position 13 in new EXTRACTION EXPRESSIONS group

* Remove typeIndex from Payload and index from Layout structs

* Update enum references in app

* Add inlined flag to Payload and rename overflown to overflow

* Propagate EVM structural changes to SDK and docs

  - Remove Operator.Nor (replaced with placeholder in EVM)
  - Remove c.nor() and c.subset() authoring functions
  - Add Operator.Empty, Operator.Slice, Operator.WithinRatio to conditionIntegrity
  - Fix checkCompValueIntegrity to handle Slice operator
  - Update documentation to reflect removed operators

* Allow Slice operator to apply to Static fields as well

* Pre-compute inlined flag in Layout during serialization

* Replace non-existent c.subset with c.every(c.or(...)) in integration tests

* Fix inlined flag computation in EIP712Encoder._toTree

* Extract and update mastercopies
