* Fix functional tool streaming response bug #262

* Fix formatting

* Fix dialyzer warnings

* Normalize streaming tool calls and preserve Zoi metadata in JSON schemas

- Normalize streaming tool calls to ReqLLM.ToolCall structs and align
tests to use ToolCall fields, ensuring follow-up requests and assertions
match non-streaming responses.

- Walk Zoi schemas to inject descriptions from meta.metadata into
generated JSON Schema (objects, arrays, enums, primitives), fixing loss
of metadata in provider formats and schema tests.
