test: add comprehensive environment variable testing for issue #17

Added test suite to verify environment variable handling:
- test-env-vars.sh: Local test script for env var detection
- .github/workflows/test-env-vars.yml: GitHub Actions workflow

Tests cover:
1. AO_ prefix environment variables
2. AUDIOBOOK_ORGANIZER_ prefix
3. --dir alias (AO_DIR)
4. Config file override
5. Docker container environment variables

Currently all tests FAIL, confirming issue #17:
Environment variables are not being recognized by the application.

The test can be run locally with: ./test-env-vars.sh
