fix(ci): separate test execution from coverage upload (#35)

- Split npm test script into test and coverage commands
- Test failures now based on actual test results, not Coveralls upload
- Coverage upload runs separately with continue-on-error
- Fixes GitHub Actions CI failures caused by Coveralls 422 errors
