fix: specify coverage file path for Coveralls upload

問題:
- カバレッジバッジが2%と誤表示される
- Coveralls GitHub Action v2 に正しいファイルパスが指定されていない

解決策:
- file パラメーターで coverage/.resultset.json を明示的に指定
- coverage-format を format に変更（v2の正しい構文）
