test: add coverage for multi-tool result message merging (#251)

Add tests verifying that consecutive tool result messages are properly
merged into a single user message for Anthropic and Bedrock APIs.

Tests added to:
- test/providers/anthropic_test.exs
- test/req_llm/providers/amazon_bedrock/anthropic_test.exs
- test/req_llm/providers/amazon_bedrock/converse_test.exs
