Add createDisjunctionParser type inference and export ParserOutput

- Add type inference to createDisjunctionParser, matching unionParser
- Export ParserOutput utility type from parser.ts and index.ts
- Replace local InferOutput/ExtractOutput types with shared ParserOutput
- Update call sites to use inferred types instead of explicit arguments
