Unify Spanned<T> support across text format crates

- Make Span/Spanned always available in facet-reflect (miette From impls
  remain behind feature flag)
- Migrate facet-json to use facet-reflect's Spanned instead of its own
- Add Spanned<T> deserialization support to facet-yaml with tests
- Add Spanned<T> deserialization support to facet-toml with tests
- All format crates now re-export Span and Spanned from facet-reflect

This allows users to wrap any field in Spanned<T> to capture source
location information during deserialization, useful for error reporting
that points back to the original source.
