feat: add __rpc.* introspection methods

- enableIntrospection option to enable introspection
- __rpc.listMethods() - list all user methods
- __rpc.describe({method}) - get method schema/description
- __rpc.describeAll() - list all methods with public schemas
- __rpc.version() - toolkit version info
- __rpc.capabilities() - server capabilities
- exposeSchema option in addMethod to make schema public
- description option in addMethod for method documentation
- Configurable prefix via introspectionPrefix (default: __rpc)
- Prevents user registration of __rpc.* methods
- Complete example in examples/introspection/
