build: Migrate to uv and update documentation (#439)

* refactor!: migrate from poetry to uv

- Replace poetry with uv for package management
- Update pyproject.toml to use PEP 621 format with dependency-groups
- Replace poetry.lock with uv.lock
- Update Makefile to use uv commands (with coverage in test)
- Update CI workflow to use astral-sh/setup-uv
- Update Dockerfile to use uv
- Update start.sh to use uv
- Modernize README.md and CONTRIBUTING.md documentation
- Add CLAUDE.md for Claude Code guidance

BREAKING CHANGE: poetry is no longer supported, use uv instead

* fix: use pytest-cov consistently for coverage reporting

Remove conflicting coverage run wrapper and use pytest-cov's built-in
lcov report generation instead.
