Consolidate duplicate CSS into shared styles.css

- Expand styles.css from 226 to 722 lines with common styles
- Remove ~1300 lines of duplicate CSS from HTML files
- Add shared components: nav, buttons, cards, tables, footer, responsive
- Add body.bg-gray class for gray background pages
- Add container.narrow class for 1000px max-width pages
- Each HTML file now contains only page-specific styles
